The Hearts in Unity way is to provide customized training and services in accordance with a formal implementation plan under the direction of the program participant, reflecting goals from the recipient’s current support plan.
We offer opportunities for adults with developmental disabilities to choose and customize services that:
Library with interdisciplinary books to enhance reading and comprehension
Arts & Craft to practice drawing, painting and craft
Computer Lab to teach computer usage, resume development and job application process
Sensory room to teach stress, relaxation, and anxiety relief techniques
Theater room to enjoy movies, music and art appreciation
Recreation room to engage in games and fun exercises to promote physical fitness
Activities of Adult Living (ADL) room to provide life skills for daily living
Companion Care to assist individuals in learning to use the library, helping with organizing volunteer activities for people, and providing assistance with specific tasks such as help with light housekeeping
Respite Care provides temporary supportive care and supervision when the primary caregiver is unable to perform these duties (under 21 years of age only)
Personal Support helps individuals with eating and meal preparation and other activities of daily living
Private Duty Nursing to provide continuous nursing care prescribed by a physician and rendered by licensed practical or registered nurses in the individual’s own home or family home
Skilled Respite where a RN or LPN/LVN will come to the home to administer medications, clean dressings, and check certain vital signs, along with other similar nursing procedures
Residential Habilitation provides specific training to improve activities of daily living
Supported Living allows many individuals, even those with severe disabilities, the ability to live in their own homes, control their lives and become a part of their community
